Birth, Death & Marriage Certificates
Vital records certificates — birth, death, and marriage — are issued by ServiceOntario, not your MPP office. But our team can help when an application is delayed, a record needs correcting, or you need a Commissioner of Oaths.
Newborn Bundle
New parents can register their baby's birth, apply for a birth certificate, get a Social Insurance Number, and apply for Canada Child Benefits — all in one online application. Available for babies under one year old. It's four transactions in one step.
Death Certificates
Order online through ServiceOntario. You'll need the deceased's name, gender, date and city of death, and parental information. Certified copies with cause of death require a separate request to the Office of the Registrar General.
Marriage Certificates
Order online through ServiceOntario. You'll need both parties' names and the date and city of the marriage, which must be registered in Ontario.
Commission Oaths
The MPP is authorized by the Province of Ontario to commission oaths, affirmations, and statutory declarations, excluding court documents. This service is used for government forms and other affidavits. Documents must be signed in person at our office.
